Dementia Support Clinics run by the Alzheimer’s Society are to start soon at Modbury Health Centre. A referral form needs to be completed in order to book an appointment – the form is available upon request from the Health Centre Reception – this is not a lengthy form but should you need help completing then please ask for assistance from one of our Reception Team. The referral can be for the person with dementia, or their carer/family or both. If dementia is affecting you or a loved one then please do seek support.
